29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection:  At the jobsite located at 9904 W. 152nd Street in Overland Park, Kansas:  Employees engaged in framing activities such as sheeting the roof were exposed to an electrical shock hazard in that ground fault circuit protection was not provided for the electrical equipment.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At the jobsite located at 9904 W. 152nd Street in Overland Park, Kansas:  Employees engaged in framing activities such as sheeting the roof were exposed to an electrical shock hazards in that strain relief was not provided at the following locations:  a)  Strain relief was not provided on the flexible cord and plug providing power to the air compressor   b)  Strain relief was not provided at the male connector end of the extension cord providing power to the rest of the electrical equipment on site.
