29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   At the residential construction site, the employer did not ensure employees installing roofing material were trained to recognize and avoid fall hazards and were exposed to fall hazards of approximately 15 feet.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  At the residential construction site, employees accessing the roof on a wooden ladder with side rails that did not extend at least 3 feet above the edge of the roof were potentially exposed to fall hazard greater than 6 feet.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.   At the residential construction site, the employer did not ensure employees installing roofing material were trained to recognize and avoid slip and fall hazards related to ladders.
