29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. Such plates shall not be removed or defaced but, if lost, removed, or defaced, shall be replaced by the owner or his agent. a) At the establishment, the mezzanine above the office, show room and rest rooms was being used for storage exposing employees from struck-by and crushing hazards should the mezzanine be overloaded. The approved loading weight of the mezzanine had not been posted

29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall: a) At the establishment employees were exposed to contact hazards when performing material handling duties. Hazards included, but were not limited to, crushing injuries to the feet, lacerations of the hands, and projectile hazards to the eyes. The employer had not conducted a workplace hazard assessment to determine the hazards of the employee job tasks and the personal protective equipment necessary to protect employees from the hazards.
