29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) Entire facility; on November 24, 2015 the employer did not maintain annual audiograms for those employees exposed at or above an 8-hour time-weighted average of 85 decibels. On December 3, 2015 Quest NoisePro DL Dosimeters recorded noise levels, for about 8 hours of sampling, that ranged from dose levels of 58.4 to 1118.1 percent on the 85 dB action level scale.

29 CFR 1910.1000(c): An employee(s) was exposed to Crystalline Silica Quartz listed in Table Z-3, in excess of an 8 hour Time Weighted Average concentration: a) Blasting area; on December 3, 2015 and times prior, the employer exposed an employee to respirable fraction crystalline silica quartz at an 8 hour calculated Time Weighted Average (TWA) of 0.273 milligrams grams per cubic meter of air which is 1.1 times the calculated 8 hour time weighted average PEL of 0.250 milligrams per cubic meter of air while assisting abrasive blasting using silica sand. Results were calculated for a 480 minute sampling period.
