29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: (a) AM General Contractors, LLC at 663 E Ridgeway Ave., Hot Sulphur Springs, CO 80451: On or before November 24, 2015, the exposing employer did not ensure that anchorages used for attachment of personal fall arrest equipment was capable of supporting at least 5,000 pounds and maintained a safety factor of at least two, in that two employees utilizing personal fall arrest systems were tied off to the same temporary roof anchor. The anchor was secured to the roof by only four nails. This condition exposed the employees to a fall hazard of approximately 12 feet while performing roofing work on a residential home.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) AM General Contractors, LLC at 663 E Ridgeway Ave., Hot Sulphur Springs, CO 80451: On or before November 24, 2015, the exposing employer did not ensure that each employee engaged in roofing operations was trained to recognize the hazards of working at elevation and trained in the use of fall protection, in that in that two employees utilizing personal fall arrest systems were tied off to the same temporary roof anchor. The anchor was secured to the roof by only four nails. This condition exposed the employees to a fall hazard of approximately 12 feet while performing roofing work on a residential home. Abatement Note: The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: a) The nature of fall hazards in the work area; b) The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; c) The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; d) The role of each employee in the safety monitoring system when this system is used; e) The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs; f) The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection; and g) The role of employees in fall protection plans; h) The standards contained in this subpart.

This condition exposed employees to a fall hazard of approximately 12 feet.
