29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a- 207 - 1rst Street, Lakewood NJ: An employee, not wearing struck by/head protection, was exposed to head injuries when working directly beneath a deck when performing stucco preparation and other related activities from a man lift basket of an articulated boom lift.  Condition observed on or about 11/23/15.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a- 207 - 1rst Street, Lakewood NJ: An employee, not wearing fall protection, was exposed to fall hazards, in excess of 20 feet to the ground surface below, when working from an elevated man lift basket of an articulated boom lift.  Condition observed on or about 11/23/15.
