29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  On or about 11/30/15; warehouse mezzanine storage area; the employer allowed employees to  to access the area to store and retrieve insulation and scrap and there were not guardrails at the open side; thus exposing employees to a fall-from- elevation hazard of 10.5'.

29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side:  On or about 11/30/15; warehouse stairs leading to mezzanine storage area;  the employer allowed employees to climb up and down a set a stairs (13 risers, 35" wide by 9" deep by 8" tall) to access insulation and scrap and the stairs did not have standard handrails on both open sides; thus exposing employees to a fall-from-elevation hazard.

On or about 11/30/15; on the mezzanine located above the first floor work area;  the employer allowed employees to store insulation and scrap on the floor and there was not a load-limit plate; thus exposing employees to an floor overload hazard.
