29 CFR 1910.253(e)(1): Equipment was not installed and used only in the service for which it was approved and as recommended by the manufacturer:      a) On or about December 2, 2015 and at times prior, employees were exposed to serious injuries from welding operations. The gauge cover was missing on the regulator of the Oxygen cylinder which was used by the employees during welding operations.

29 CFR 1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv):      a) On or about December 2, 2015 and at times prior, employees were exposed to health illness while handling chemicals in the workplace. The employees were not provided with the training required in the hazards communication program. The employees where handling chemicals to include but not limited to the following chemicals:    1. Sherwin-Williams Toluene - contains Toluene (can cause adverse symptoms such as nervous system depression or death)   2. Berryman carburetor and parts cleaner - contains Ethanol (can cause adverse symptoms such as headache, nausea, and vomiting)  3. Quaker State brake fluid cleaner - contains Glycol Ether (can cause adverse symptoms such as drowsiness, dullness, numbness, and headache)
