29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Fabrication area: The AccurPress press brake (725012) was not equipped safeguards to prevent contact by the operator or helper with the point of operation of the machine, as observed on or about December 2, 2015.  b) Fabrication area: A portion of the guard on the Adira shear (GHO-0630) was missing allowing access to the point of operation of the machine, as observed on or about December 2, 2015.  c) Fabrication area: The Chicago hydraulic press was not equipped with safeguards to prevent contact with the point of operation of the machine, as observed on or about December 2, 2015.  d) Fabrication area: Work stations on the Piranha ironworker (P50), including the notching/coping, punch and drop off side of the flat shear, were not equipped with safeguards to prevent contact with the points of operation of the machine, as observed on or about December 2, 2015.  e) Fabrication area: The Verson press brake (B-78) was not equipped with safeguards to prevent contact with the point of operation of the machine, as observed on or about December 2, 2015.  f) Low bay area: The AccurPress press brake (71008) was not equipped with safeguards to prevent contact with the point of operation of the machine, as observed on or about December 2, 2015.
