29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At 1325 E Jackson Blvd in Jackson, MO 63755 - On or about 11/12/2015, each employee engaged in residential construction activities, approximately 15 feet or more above a lower concrete surface, was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were the employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 1325 E Jackson Blvd in Jackson, MO 63755 - On or about 11/12/2015, where employees were using a portable ladder to access an upper landing surface and the ladder's length allowed, the ladder side rails extended 2 and 1/2 feet above the upper landing surface being accessed.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
