29 CFR 1910.132(a): Protective equipment was not provided and used wherever it was necessary by hazard of processes:    a) 100 Block West Church Street, Williamsport: The employee was exposed daily to struck-by hazards from motor vehicle traffic while conducting waste/refuse collection activities within or near roadways during periods of darkness and reduced visibility without a Class 3 high visibility apparel ensemble (e.g. combination of Performance Class E trousers and Performance Class 2 vest or Performance Class 3 vest), on or about December 2, 2015.    In accordance with 29 CFR 1903.19(c) and (d), abatement certification is required for this violation.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment.  a) 100 block of West Church Street, Williamsport, PA - A personal protective equipment hazard assessment was not conducted by the employer for employees exposed to laceration, struck-by, burns and other hazards created by metal, broken glass, sharps, flying particles, potentially hazardous chemicals and other materials while collecting commercial and residential refuse, on or about December 2, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
