29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: (a). Worksite located at 1503 S. Mumm Ave., Maryville, Missouri- Employees were exposed to a shock or electrocution hazard in that an extension cord was run out of the contractor's trailer and was plugged into a pump that was submerged in water and was not protected by the use of a GFCI.

29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: (a). Worksite located at 1503 S. Mumm Ave., Maryville, Missouri- Employees were exposed to a shock or electrocution hazard in that an extension cord was run through an aluminum framed window in which the frame was pinching the cord and could cause the cord's outer insulation to be breached and energize the window frame.
