29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:     On or about December 3, 2015, shopping center parking lot, an employee in the elevated basket of a truck mounted articulating boom lift did not wear a body harness and lanyard attached to the boom when working at heights greater than 4 feet above the ground which exposed the employee to fall hazards.
