29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a.)    Willis CNC, LLC. located at 1008 Commerce Dr. Grafton, OH; On or about December 3, 2015, the employer did not assure an energy control program, consisting of energy control procedures, training and periodic inspections was provided to employees. Employees are required to perform setup and maintenance activities on the Ganesh CNC Machines, including but not limited to #5, 7, 30, 31, and 32 and are exposed to caught by and amputation hazards related to unexpected start up of the equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a.)    Willis CNC, LLC. located at 1008 Commerce Dr. Grafton, OH; On or about December 3, 2015, the employer did not assure employees were protected while operating the Ganesh CNC machines. Employees are required to use, including, but not limited to, #5, #7, #30, and #32 Ganesh CNC machines and are exposed to caught by and amputation hazards from the rotating tooling in that the door can be opened during the cycle.

Willis CNC, LLC. located at 1008 Commerce Dr. Grafton, OH; On or about December 3, 2015, the employer did not assure employees were protected while working in the Tool Room. Employees are required to access the Tool Room periodically throughout the day and are exposed to struck by hazards in that the overhead storage area above the Tool Room is not rated for its capacity.
