29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:  Jobsite: On, or about, 12/07/2015 employees were engaged in construction activity from a ladder-jack scaffold and exposed to a fall of approximately 14 feet to the lower level whereby they were not protected by a guardrail system, net system, or personal fall protection system.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Jobsite: On, or about, 12/03/2015 employees were installing siding to the chimney structure and exposed to a fall of approximately 14 feet to the lower level whereby they were not protected by a guardrail system, net system, or personal fall protection system.
