29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   On or about December 9, 2015, during the installation of siding at a job site located at 7453 Montella Ave NW, Massillon, Ohio 44646; the employer did not ensure employees working on a ladder jack scaffold were protected from a fall of approximately 13.7 feet by utilizing a fall protection system.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:   On or about December 9, 2015, during the installation of siding material at a job site located at 7453 Montella Ave NW, Massillon, Ohio 44646; the employer did not ensure portable ladders were utilized in the manner in which the ladder was designed. The company separated a 20 foot portable ladder (extension ladder) and utilized half of it to support a ladder jack scaffold.
