29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation: On or about December 14, 2015, where employees were exposed to a drowning hazard when operating a tracked excavator near the edge of a water-filled excavation that was not barricaded.
