29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) 5212 Arendell Ave, Philadelphia, PA - The employer did not ensure that the employees were using some type of fall protection system when performing roofing operations on a steep roof thus exposing the employees to a fall hazard. This condition was observed on or about December 10, 2015.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     a) 5212 Arendell Ave, Philadelphia, PA - The employer did not ensure that the employees using the extension ladder allowed the ladder side rails to extend 3 feet above the upper landing surfaced being accessed and the ladder was not secured at its top to a rigid support. This hazard exposed the employees to a fall hazard. This condition was observed on or about December 10, 2015.
