29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) On or about December 14, 2015, in a material storage area, an employee using a cutting torch to remove a boom support stand from a truck-mounted hole digger was exposed to a crushing hazard due to the release of stored gravitational energy.  The employer had not established a program consisting of energy control procedures, employee training, and periodic inspections to ensure isolation of energy sources prior to performing servicing and/or maintenance activities.     In the alternative    Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard:    (a) On or about December 14, 2015, in a material storage area, an employee using a cutting torch to remove a boom support stand from a truck-mounted hole digger was exposed to a crushing hazard due to the release of stored gravitational energy.
