29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    On or about December 15, 2015, and times prior to, the following forklifts were not outfitted with a seat belt:    (a)  The #5 Daewoo forklift located in the Maintenance Shop that was used for picking up and unloading freight.  (b)  The #4 Toyota forklift located in the Specialty Shop that was used for transporting parts.    This condition exposed employees to the hazard of being thrown, ejected and crushed in the event of a tip-over.    (c)  Employees are not provided with a face shield and apron when refueling LP cylinders for forklifts.  This condition exposed employees to contact with the fuel.
