29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about December 15, 2015:  a) An employee working on roofing system was exposed to fall hazards of approximately 10.6 feet when the employer failed to ensure that the employee was properly protected form the fall by the use of fall protection measures.   No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about December 15, 2015:  a) Employees were exposed to fall hazards of approximately 10.6 feet when the employer failed to ensure the extension ladder being used to gain access to the upper landing had its rails extending at least three feet above the upper landing surface.  No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  On or about December 15, 2015:  a) Employees were exposed to fall hazards when the ladder they were using had damage to the rungs on the ladder.  No abatement certification or documentation is required for this item.
