29 CFR 1910.178(l)(6): 1910.178(l)(6):  Located at Hampton VA Medical Center, the employer had not certified that the industrial truck operator had been trained and evaluated in the use of powered industrial trucks as required by paragraph (l) of 1910.178, such as, but not limited to, employee operating the Toyota 4500 pound capacity electric truck.  (a) On or about 12-17-2015 and times prior thereto at Hampton VA Medical Center, Building 27 Warehouse, 100 Emancipation Drive, Hampton, VA 23667, The employer did not certify that an employee who was operating a Toyota 4500 pound capacity, Model #7HBW23, Serial #7HBW23-41095, Type "E" Electric Truck was trained or evaluated as being competent to do so.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
