29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: (Construction Reference 1926.103):  On or about December 15, 2015 at 4300 West 7th St., Little Rock, AR,  for the benefit of employees performing tasks such as chipping concrete containing silica and mixing Quikrete containing Portland cement and silica sand,  the employer had not evaluated the respiratory hazards to include a reasonable estimate of the employee's exposure to respirable dust containing silica.  Employees were provided 3M N95 disposable respirators for voluntary use.
