29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work on, had the strength and structure integrity to support employees safely:  On or about October 29, 2015, the employer allowed employees to work on the mezzanine area that had not been assessed to determine its ability to support the weight of the employees. An employee was working on the mezzanine and fell through the drop ceiling tiles into the hallway.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  An employee was exposed to fall hazards while installing cabling for a closed-circuit security system and working from a mezzanine in the warehouse area. The employee was not provided with training to recognize and minimize fall hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  Employees installing cabling for a closed-circuit security system in a warehouse were required to use ladders and were not trained in the proper use, handling or hazards associated with ladders.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
