29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: In the seat foam mold area of the foam plant located at 4000 NW 44th Street, Lincoln, NE 68520, employees are exposed to respiratory hazards including but not limited upper respiratory tract irritation while voluntarily using filtering face piece respirators for comfort from nuisance dust particles in that; respirators are stored by them hanging from the straps in the mold release area where they can be contaminated by particulates.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: In the seat foam mold area of the foam plant located at 4000 NW 44th Street, Lincoln, NE 68520, employees are exposed hazards including but not limited to being struck-by or caught-in the foam injection robot, and pulmonary edema and asthma from exposure to isocyanates mists in that; the employer did not ensure that the pressurized isocyanate fluid lines and foam injection robot was deenergized, isolated and locked out prior to employees being permitted to enter the robot cage to troubleshoot and service a leaking hose.
