Employees were using a metal wall box as an extension cord. Wall box was not mounted as in accordance with manufacturers use.

Employees were using an extension cord that had the energized conductor exposed and protruding because of damage to the cord insulation.

Employees were standing on the toe rail edge of the basket while using a rope to lift materials up to the basket. Employees exposed to a fall hazard of approximately 24 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) For employees working at 429 48 Court Street East, Building 11 in Bradenton, as observed on or about December 16, 2015. Employees were working on an unguarded balcony installing siding without any fall protection in use. Employee was exposed to a fall of approximately 21 feet 5 1/2 inches.
