29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative:    a) At the facility an energy control program that consisting of employee training, energy control procedures and periodic inspections had not been implemented.  Employees are exposed to high speed cutting tools and rotating parts which could cause amputation and other serious injuries when performing maintenance tasks.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) The safety interlock barrier guard of the VMC Mazatrol CNC Lathe, Model VCN510C-II, Serial # 244102 was bypassed, exposing employees to rotating parts, ingoing nip points and flying chips.     b) The safety interlock barrier guard of the VMC Fadal 4020 CNC Mill, Model 906-1, Serial # 9707581 was bypassed, exposing employees to rotating parts, ingoing nip points and flying chips.     c) The safety interlock barrier guard of the VMC Fadal 15 CNC Mill, Model 914-15, Serial # 9709002 was bypassed, exposing employees to rotating parts, ingoing nip points and flying chips.     d) ) The safety interlock barrier guard of the VMC Hurco 40 CNC Mill, Model VMX40, Serial # H442-01014120AFA was bypassed, exposing employees to rotating parts, ingoing nip points and flying chips.

29 CFR 1910.212(a)(3)(iv)(e): One or more methods of machine guarding were not provided to protect the operator and other employees in the manual milling area of the shop from the point of operation:    a) The Mazatral T Plus CNC lathe, Model M5N, Serial number 128982 M5N  was not provided with machine guards, thereby exposing employees to being caught in hazards.    b) The Mazac Nexus 200 CNC lathe, Model QTN-200, Serial number 162477 QTN-200 , was not provided with machine guards, thereby exposing employees to being caught in hazards.    c) The Mazac Nexus 350 CNC lathe, Model QTN-350, Serial number  205121 QTN-350,  was not provided with machine guards, thereby exposing employees to being caught in hazards.    d) The Mazac Nexus 300 CNC lathe, Model QTN-300, Serial number  172883 QTN-300,  was not provided with machine guards  thereby exposing employees to being caught in hazards.    e) The Mazac Nexus 300 CNC lathe, Model QTN-300, Serial number  215275 QTN-300,  was not provided with machine guards, thereby exposing employees to being caught in hazards.    f) The Leblond manual lathe, Model 12-E 639, Serial number  EE4255,  was not provided with machine guards thereby exposing employees to being caught in hazards.    g) The Kingston manual lathe, Model HD-22120, Serial number CH00839 was not provided with machine guards, thereby exposing employees to being caught in hazards.     h) The Ganesh manual lathe, Model GT-2080, Serial number  CC9607-008,  was not provided with machine guards, thereby exposing employees to being caught in hazards.    i) The Herbert manual lathe, Model DR-55, Serial number 16T6974,  was not provided with machine guards, thereby exposing employees to being caught in hazards.    j) The Herbert manual lathe, Model DR-55, Serial number 14T12771,  was not provided with machine guards, thereby exposing employees to being caught in hazards.
