29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, a. Located in the Heavy Check area of the hanger at 4330 East Fifth Avenue in Columbus, Ohio: On or about December 16, 2015 and at times prior, the employer did not ensure that an adequate means of fall protection, such as guardrails, were maintained while loading flat carts from the heavy check platforms, exposing employees to an approximately eight foot fall hazard.

29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. a. Located in the Heavy Check area of the hanger at 4330 East Fifth Avenue in Columbus, Ohio: On or about December 16, 2015, employees were offloading fully serviced gallery carts from the heavy check platform and placing them onto an unstable rolling flat cart which was raised to the elevation of the platform on the forks of a propane powered Toyota forklift, Model 42 6FGU25, Serial 63486. The flat cart was not secured to the forks of the forklift and extended beyond the wheel base of the forklift by approximately twenty-eight inches, exposing employees to a struck by hazard.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle.
