29 CFR 1926.50(g): Where employees were exposed to injurious materials. suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:       a) Prime Hook National Wildlife Preserve, Broadkill Beach Road, Milton, DE - On or about December 22, 2015, the employer failed to provide adequate emergency eyewash station (a North solution bottle provided at site expired in November of 2008) for employees mixing and using fuels including gasoline and diesel and cleansers, exposing employees to eye/face injuries and chemical hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:     a) Prime Hook National Wildlife Preserve, Milton, DE - On or about August 31, 2015, the employer failed to report an amputation of an employee who tripped and amputated part of a finger while conducting dredging activities.  The amputation was reported as part of a complaint on December 18, 2015.
