29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5): World Class Distribution, Inc. warehouse at 1510 Cargo Ct., Minooka, IL: On or about October 2, 2015, and thereafter, powered industrial truck operators were not provided with adequate training prior to being assigned to drive a different type of truck including, motorized pallet rider truck (electric pallet jack), man-up stock picker lift (cherry picker), reach truck, and sit-down counterbalance truck as required by paragraph (l). Training on established formal instructions and practical training and exercises were not completed for each operator. Employees were exposed to struck-by hazards likely to result in serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees were exposed to struck-by hazards likely to result in serious injury. (b) World Class Distribution, Inc. warehouse at 1510 Cargo Ct., Minooka, IL: On or about December 22, 2015, for each recordable case in OSHA 300 Log, the employer did not describe injury and/or illness, and object/substance that directly injured or made person ill. (c) World Class Distribution, Inc. warehouse at 1510 Cargo Ct., Minooka, IL: On or about December 22, 2015 the employer did not enter correctly the October 2, 2015 work-related injury on the OSHA Form 300 for the calendar year 2015.
