The employer fails to assure effective employee fall protection as required by the standard, as demonstrated on July 31, 2015 and December 29, 2015, when employees working from residential roofs  with a height greater than 6-foot at a workplaces located at 109 Southern Grove Drive, Saint Johns, Florida 32259 and 1625 Derringer Road, Jacksonville, Florida 32225
