29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Facility - Production Area: On, or about, 12/21/2015 employee conducts maintenance on machinery and other employees operate machines, without an energy control program in place to ensure that procedures, training and periodic inspections were implemented for the prevention of injuries caused by the released of stored energy, including electrical and pneumatic sources.    Machines included, but may not be limited to:  1)         Inserter Machine, 6 x 9 - Aisle # 8;    2)         Inserter Machine, 6 x 9 - Aisle # 9;    3)         Inserter Machine, 6 x 9 - Aisle # 10;    4)         Inserter Machine, 6 x 9 - Aisle # 11.   Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a) Facility - Production Area: On, or about, 12/21/2015 the employer did not ensure electrical equipment attached to inserter machines were enclosed. Machines included, but may not be limited to:    1)         Inserter Machine, 6 x 9  Aisle # 10 and;    2)         Inserter Machine, 6 x 9  Aisle # 11.   Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
