29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A-D): The employer does not ensure that energy control procedures identify the specific steps to follow to isolate and control hazardous energy of machines during servicing and maintenance activities. The employers lockout procedures did not cover: (1) The magnitude of the electrical and compressed air energy to be controlled. (2) Procedural steps for an orderly shutdown of the machine. (3) Procedures for the application of lockout devices. (4) Procedures for the release of stored energy. (5) Specific steps for verification that isolation and deenergization has been accomplished. Employees working to decommission the #4 Komax machine were not provided energy control procedures with specific isolation and control steps and were exposed to the hazard of electrical shock.

29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating devices and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of action was not followed: Employees who performed testing or positioning of machines and equipment were not required to follow the required sequence of actions: (1) Clear the machine or equipment of tools and materials in accordance with paragraph (e)(1) of this section, (2) Remove employees from the machine or equipment area in accordance with paragraph (2) of this section, (3) Remove the lockout or tagout device as specified in paragraph (3) of this section, (4) Energize and proceed with testing or positioning, (5) Deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue the servicing and maintenance. Employees required to rely upon electrical control components for energy control were exposed to electric shock hazards from unexpected start-up and energizing of the #1 Direct Connect Machine during a program change.
