29 CFR 1926.451(c)(2)(iv): Front-end loader and similar pieces of equipment were used to support scaffold platforms when equipment was not specifically designed by the manufacturer for such use:  a) 1000 Hoes Lane, Piscataway, NJ 08854. The employer allowed his employees to work on and from a job-made elevated work platform that was not specifically designed and or approved by the manufacturer for such use exposing the employees to fall hazards.   Condition occurred on or about 12/10/15.

29 CFR 1926.451(c)(2)(v): Fork-lift was used to support scaffold platform when the entire platform was not securely attached to the forks and when occupied was moved horizontally:   a) 1000 Hoes Lane, Piscataway, NJ 08854. The employer allowed his employees to work on and from a job-made elevated work platform that when occupied was moved horizontally and not properly secured to the forks.    Condition occurred on or about 12/10/15.

29 CFR 1926.451(g)(4): Guardrail systems installed did not meet the requirements of this section and did not comply with the provisions set forth in Appendix A of this subpart deemed to meet the requirements of paragraphs (g)(4)(vii), (viii) and (ix) of this section:  a) 1000 Hoes Lane, Piscataway, NJ 08854. The employer allowed his employees to work on and from a job-made elevated work platform when the guardrail systems did not meet the requirements in paragraphs (g)(4)(vii), (viii) and (ix). The cable being used for the top rail was installed at the wrong height and the cable had a sag distance in excess of six inches.   Condition occurred on or about 12/10/15.
