29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  (a) On or about December 29, 2015, and at times prior, the employer did not ensure that each power industrial truck operator was competent to operate a forklift safely, as demonstrated by successful completion of training and evaluation of such industrial trucks. Warehouse personnel were allowed to operate a Daewoo G30S propane forklift, without the proper training and evaluation required to operate forklifts.     Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

Warehouse personnel were permitted to ride on the side of a Daewoo G30S propane forklift, exposing employees to struck-by hazards. 29 CFR 1910.215(a)(1): Abrasive wheel(s) used on grinding machinery were not used with safety guard(s):  (a) On or about December 29, 2015, and at times prior, the employer did not ensure that abrasive wheels used on grinding machinery were used with safety guards. Employees in the wood shop were exposed to struck-by hazards, in that a bench grinder was used without the required safety guards for the abrasive wheel.
