29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches:     Wiskerchen Truck and Equipment LLC facility in Spencer, WI: On and before  October 8, 2015, employees were exposed to struck by hazards while hoisting truck parts with an overhead crane.  The rigging method of only using two synthetic slings (model TF2904X8) would not maintain balanced load during lift.

29 CFR 1910.184(c)(6): Slings were not securely attached to their loads:    Wiskerchen Truck and Equipment facility in Spencer, WI: On and before October 8, 2015, the employer did not ensure that the hooks on the synthetic slings (model TF2904X8) maintained a positive connection to the fabricated metal truck parts being lifted.
