29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity:    On or about 12/30/2015, in the food Deli, near a work table, located behind the food display, an electrical outlet had it's ground conductors attached to the hot terminal, thus, creating reverse polarity. The condition exposed the workers to electrical hazard while plugging hand held tools such as but not limited to mixers.
