The employees were exposed to a potential fall hazard of up to approximately 12 ft. to the lower floor; on or about 12/30/15.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Location: 717 Flushing Ave., Brooklyn, NY. a) Employees were observed working from a tubular welded scaffold without any means of fall protection. The scaffold did not have a guardrail system and was not fully planked at the working levels. The employees were exposed to a potential fall hazard of up to approximately 12 ft. to the lower floor; on or about 12/30/15.
