While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings:  a) Pinnacle Stone Products, LLC (exterior, South/Southeast corner) - The employer did not ensure that a sludge pit approximately 8ft long X 3ft wide and 4ft deep was guarded by a floor opening cover of standard strength and construction.  The pit's hose entry approximately was not covered and the pit was not constantly attended by someone nor was it protected on all sides by removable standard railings.  Observed on or about January 6, 2016.
