29 CFR 1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   (a)   Pine Bluffs Gravel & Excavating, Inc. at 220 North Ord Street, Grover CO 80729:  On January 6, 2016 the exposing employer did not ensure that a stairway, ladder, ramp or other safe means of egress was provided to employees that worked in the north trench, in that employees accessed the trench by walking the sides of the trench.  The trench was approximately 9 feet in depth, 4 feet in width at the bottom, and 35 feet in length.  This condition did not provide employees with a safe means of trench egress.

The north trench was approximately 9 feet in depth, 4 feet in width at the bottom, and 35 feet in length.  This condition exposed the employees to the hazard of spoil pile material falling into the excavation or causing the excavation wall to collapse.      (b)  Pine Bluffs Gravel & Excavating, Inc. at 220 North Ord Street, Grover, CO 80729:  On January 6, 2016 the exposing employer did not ensure that adequate protection was provided, in that the spoil pile from the south trench was placed at the edge of the opening.  The south trench was approximately 10 feet in depth, 4 feet in width at the bottom, and 43 feet in length.  This condition exposed the employees to the hazard of spoil pile material falling into the excavation or causing the excavation wall to collapse.

29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):   (a)   Pine Bluffs Gravel & Excavating, Inc. at 220 North Ord Street, Grover, CO 80729:  On January 6, 2016 the exposing employer did not ensure that each employee repairing a water main inside the south trench was protected from cave-in, in that the slope of the east side near the south end measured approximately 50 degrees.  The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i), in that the maximum allowable slope associated with Type B soil is an angle of 45 degrees, from toe to top.  The trench was approximately 10 feet in depth, 4 feet in width at the bottom, and 43 feet in length.  This condition exposed employees to a crushing hazard by excavation wall cave-in.
