29 CFR 1926.96: The employer did not ensure employees wear proper footwear which meets the requirements and specifications in American National Standard for Men's Safety-Toe Footwear, Z41.1-1967.     a. On December 15, 2015, the employer allowed three employees to work on the construction site and carry building materials while working on the roof using a drill without wearing hard sole safety shoes. All three employees were wearing casual sandals.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   a. On December 15, 2015, on the east side of the construction site, employees were observed working on the ground level without wearing hard hats while work was being performed above them on the roof and on the scaffold. Employees on the ground level were exposed to overhead struck-by hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a. On December 15, 2015, on the east side of the construction site, employees working on the roof without guard railings or personal fall arrest systems and were exposed to a 12 to 20 foot fall hazard to the ground level below.
