Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: The employer is failing to protect employees from fire and explosion hazards while they are engaged in oil tank cleaning and maintenance activities. This was most recently documented as occurring on or about December 30, 2015 at Section 18, Range 38,Township 30 Ulysses, Kansas, where sources of ignition were not controlled or eliminated such as but not limited to static.
