29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary:     Century Tube Corporation/Somerville, NJ- Manufacturing Area: Employees whom annual audiograms indicated that they experienced Standard Threshold Shift (STS), and who wear hearing protection such as 3M EARsoft ear plugs were not refitted and retrained in the use of hearing protectors.    Condition observed on or about January 7, 2016.

29 CFR 1910.95(i)(5): The employer did not ensure proper initial fitting and supervise the correct use of all hearing protectors:    Century Tube Corporation/Somerville, NJ- Manufacturing Area: The employer did not ensure that employees who are required to use hearing protectors including those who experienced Standard Threshold Shift (STS) correctly use them. Employees were observed improperly wearing their hearing protection such as, but not limited to 3M ear plugs.  The ear plugs were not fully inserted into the ear canal.     Condition observed on or about January 7, 2016.

29 CFR 1910.1200(h)(3)(iv):  The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:   Century Tube Corporation/Somerville, NJ- Manufacturing: Employees were not provided with information and training on Globally Harmonized System of Classification and Labeling of Chemicals, including new label elements and the new standardized format for Safety Data Sheets.  Employees use hazardous chemicals such as but not limited to lubricants on daily basis.   Condition observed on or about January 7, 2016.
