29 CFR 1910.178(l)(1)(i):       The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):      (a) On or about December 30, 2015 at worksite 4020 E 138th, Grandview, Missouri.- employees were exposed to a struck-by and crushing hazard when operating a Caterpillar Model V500 propane fueled powered industrial truck without  successful completion of required training and evaluation.      Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.178(p)(1):      Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s)        (a) On or about December 30, 2015 at worksite 4020 E 138th, Grandview, Missouri.- employees in the warehouse area were exposed to a serious struck by hazard from the Caterpillar Model V500 propane fueled powered industrial truck in operation, without the rubber cover on the clutch pedal.           Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
