29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually:       (a)  Holiday Retirement at 3260 N. 12th St., Grand Junction, CO 81506: On and before    January 13, 2016, the employer did not review and update the Exposure Control Plan for the establishment on an annual basis to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure. Employees were occupationally exposed to blood or Other Potentially Infectious Materials (OPIM), in that employees, as part of their job duties, cleaned spills of blood and collected linens soiled with dried blood. This condition exposed employees to a bloodborne pathogen hazard.

29 CFR 1910.1030(c)(2)(i)(B): The exposure determination did not include a list of all job classifications in which employees have occupational exposure:    (a)  Holiday Retirement at 3260 N. 12th St., Grand Junction, CO 81506: On and before  January 13, 2016, the employer did not prepare an exposure determination in the Exposure Control Plan which listed job classifications for employees that have occupational exposure.  Employees were occupationally exposed to blood or Other Potentially Infectious Materials (OPIM), in that employees, as part of their job duties, cleaned spills of blood and collected linens soiled with dried blood. This condition exposed employees to a bloodborne pathogen hazard.
