29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:    (a)  Blackhawk Construction Company, LLC at 1273/1275 Ogden St, Denver, CO 80204:  On or before December 11th, 2015, the exposing employer did not ensure that employees were protected from injury, in that six employees removing shingles from a two story house, with a roof pitch of 7-in-12, were not utilizing a fall protection system when working at and near the edge.  This condition exposed employees to fall hazards of approximately 10 and 20 feet.
