29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.  On or about January 7, 2016, employees were observed working on mobile scaffold that were not fully planked when performing framing at the height of 9 feet or more above the concrete surface, exposing employees to fall hazards.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  On or about January 7, 2016, wheels were not locked when employees were working on mobile scaffold, exposing employees to fall hazards at heights of approximately 9 feet or more above the concrete surface.
