29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provided for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    (a) Jazz It Up w/discount construction, at 2621 W 34th Avenue, Denver, CO: On and preceding 1/12/16, the employer did not initiate and maintain a safety program which provided for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Employees of Jazz It Up w/discount construction were remodeling a two story home.  The employees were exposed to fall hazards while performing roofing work, while working from ladders, and while working upon scaffolding.  Fall protection, ladders, and a job built scaffold were not assembled in accordance with accepted standards.  The employer did not perform inspections of the worksite and equipment.  This condition exposed employees to approximate 16 and 20 foot fall hazards.
