29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons shall be adequately trained to render first aid. Adequate first aid supplies shall be readily available.      a) The employer did not ensure that employees were adequately trained to render first aid to employees receiving serious and life threating injuries. On November 25, 2015 employees injured when three bales of pads fell on them did not receive adequate first aid treatment in that 911 was not called for 38 minutes to render first aid and an injured employee drove to the hospital to receive treatment.

29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.    a) On November 25, 2015 the bundles of pad material size 30 by 44 by 57 inches with weights ranging from 521 to 640 pounds were not stacked, blocked, interlocked and limited in height. They were stacked 3 bundles high allowing the bales to fall onto the employees located in the bales storage area while they were removing the pads exposing them to struck by hazards due to the unsecured storage.
