Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  (a)   Eastside of the building - On or about On or about January 7, 2016 three employees were exposed a fall hazard while riding in a Genie S-40 Boom Lift elevated 16 feet above the ground without fall protection.  Also this aerial lifts occupancy capacity was exceeded. This aerial lift has maximum occupancy of 2 passengers.

(a)   Eastside of the building - On or about On or about January 7, 2016 three employees were exposed a fall hazard while riding in a Genie S-40 Boom Lift due to an exceeded occupancy capacity. This aerial lift has a warning sticker on the front that reads Maximum of 2 Occupants or 500 lbs. All three of these employees were elevated 16 feet above the ground without fall protection. The operators manual also states maximum capacity of 2 occupants.
